For Participation in One FC Boston Tryouts

Effective Date: March 9, 2026

1. Parental Consent and Authorization

I/We, the undersigned, hereby certify that I/We am/are the parent(s) or legal guardian(s) of the minor participant (“the Player”). By completing the registration, I/We agree to the following:

a) Authorization to Participate

I/We authorize the Player to participate in the One FC Boston Tryouts, organized and operated by VISION FC BOSTON (“THE CLUB”), including all related athletic activities conducted on or off the designated field premises.

Participation is voluntary and may involve physical exertion and competitive activities.

2. Media Consent

a) Image Rights

I/We understand that THE CLUB may capture photographs, video footage, livestreams, or audio recordings of the Player during tryouts, training sessions, or related activities.

b) Authorization of Use

I/We grant VISION FC BOSTON and its authorized representatives the unrestricted, royalty-free, worldwide right to use the Player’s image, name, likeness, and voice, in any format or media now known or developed in the future.

These materials may be used for purposes including but not limited to:

- Marketing and promotional materials

- Social media content

- Websites and digital platforms

- Advertising campaigns

- Event promotion and club communications

This authorization is granted without compensation or additional consent.

c) Ownership

All photographs, videos, recordings, and media content produced remain the exclusive property of THE CLUB.

d) Liability of Third Parties

In the event a third party misuses such content outside the intended scope of this agreement, such third party shall bear full liability, and THE CLUB shall be held harmless.

3. Assumption of Risk and Liability Waiver

a) Participation Risks

I/We acknowledge and understand that participation in soccer activities involves inherent risks, including but not limited to:

- Physical contact with other participants

- Falls, collisions, or impacts

- Weather and field conditions

- Physical exertion that may lead to injury or illness

These risks may result in serious injury, permanent disability, or other health complications.

b) Assumption of Risk

I/We voluntarily assume all risks associated with the Player’s participation in the One FC Boston Tryouts.

c) Waiver of Liability

To the fullest extent permitted by law, I/We, on behalf of ourselves and the Player, waive, release, and hold harmless VISION FC BOSTON, its owners, directors, staff, coaches, volunteers, sponsors, partners, and affiliated facilities from any and all claims, demands, liabilities, damages, or expenses, including those arising from negligence, related to the Player’s participation in the tryouts.

4. Medical Treatment Authorization

I/We authorize representatives of VISION FC BOSTON to seek appropriate medical attention for the Player in the event of an injury, illness, or emergency occurring during participation in the tryouts.

I/We acknowledge and agree that:

- I/We are responsible for maintaining appropriate medical and health insurance coverage for the Player.

- Any medical treatment or services provided will be at our own expense.

5. Weather and Safety Policy

In the event of inclement weather or unsafe conditions, all participants will be directed to designated safe areas.

THE CLUB reserves the right to:

- Delay

- Reschedule

- Modify

- Cancel sessions

due to weather, safety concerns, or unforeseen circumstances.

As the tryouts are provided free of charge, no refunds or reimbursements apply.

6. Data Privacy

a) Data Collection

By submitting the registration form, I/We acknowledge that personal data of both the parent/guardian and the Player will be collected and processed by VISION FC BOSTON for the purpose of organizing and managing participation in the tryouts.

b) Use and Storage

This data may be used to:

- Communicate tryout information

- Track participation and attendance

- Provide updates about club programs and opportunities

Data will be stored securely and will not be sold or shared with third parties, except authorized service providers acting on behalf of VISION FC BOSTON.

c) Rights

7. Final Declaration

By completing the registration process, I/We confirm that:

- The information provided is accurate and complete

- I/We have read and understood these Terms and Conditions

- I/We voluntarily agree to all provisions outlined in this document

and consent to the Player’s participation in the One FC Boston Tryouts.
